Diabetes

Overview

Diabetes means the body has trouble keeping blood sugar (glucose) in a healthy range. In type 1 diabetes, the pancreas makes little or no insulin, usually starting in childhood or young adulthood. In type 2 diabetes — the far more common form in adults — the body doesn't use insulin well and, over time, may not make enough. Left unmanaged, high blood sugar gradually damages blood vessels and nerves throughout the body.

Key point: Diabetes is very manageable. Most people with diabetes live full, active lives — the goal of treatment is steady blood sugar control, not perfection.

Diagnosis & Screening

Diabetes is diagnosed with a blood test — most commonly the A1C, which reflects average blood sugar over about three months, or a fasting glucose test. An A1C of 6.5% or higher on two occasions, or a fasting glucose of 126 mg/dL or higher, generally confirms the diagnosis. An A1C between 5.7% and 6.4% indicates prediabetes, a stage where lifestyle changes can often prevent progression.

Adults are generally screened starting at age 35, or earlier if there's a family history, higher body weight, or other risk factors. Ask your care team when your next screening is due.

Treatment

  • Type 1: Lifelong insulin, delivered by injection or pump, matched to food intake and activity.
  • Type 2: Often starts with lifestyle changes and a first-line medication (commonly metformin), with additional medications added as needed — including newer options that also support weight and heart health.
  • Both types: Regular follow-up to review blood sugar trends, medication side effects, and screening for complications.
Ask your clinician: what your individual A1C target is — it isn't the same for everyone, and depends on age, other health conditions, and how long you've had diabetes.

Monitoring at Home

Depending on your treatment, your care team may recommend a fingerstick glucose meter, a continuous glucose monitor (CGM) worn on the skin, or periodic lab checks alone. Keeping a simple log — even a few readings a week — helps your care team fine-tune treatment between visits.

Diet & Daily Living

  • Consistent meal timing and portion sizes, rather than strict food elimination
  • Favoring whole grains, vegetables, and lean protein over refined carbohydrates and sugary drinks
  • Regular physical activity — even short daily walks measurably improve blood sugar control

Seek urgent care if you experience: confusion, difficulty waking, fruity-smelling breath, persistent vomiting, or a blood sugar reading far outside your usual range along with feeling unwell. These can signal a medical emergency.

Older & Homebound Adults

Diabetes management often shifts for older adults, especially those with multiple conditions or limited mobility. Blood sugar targets are frequently relaxed to reduce the risk of dangerous lows (hypoglycemia), which are often more harmful than modestly elevated readings. Caregivers play a central role in medication management, meal support, and recognizing early warning signs. If leaving home for appointments is difficult, ask whether a home-based primary care program is available in your area.

Common Questions

Can type 2 diabetes go away? For some people, significant weight loss and lifestyle change can bring blood sugar into a normal range without medication — often called remission. It requires ongoing maintenance and isn't guaranteed for everyone.

Is insulin a sign I'm "failing" at managing my diabetes? No. Type 2 diabetes often progresses over time regardless of effort, because the pancreas gradually makes less insulin. Needing insulin is a change in the disease, not a personal failure.

Overview

Blood pressure is the force of blood pushing against artery walls. When it stays elevated over time, it strains the heart and blood vessels — often without any noticeable symptoms, which is why hypertension is sometimes called a "silent" condition. Uncontrolled high blood pressure is a leading contributor to heart attack, stroke, and kidney disease.

Diagnosis & Numbers

Blood pressure is written as two numbers: systolic (the higher number, pressure when the heart beats) over diastolic (the lower number, pressure between beats).

  • Normal: below 120/80
  • Elevated: 120–129 systolic and below 80 diastolic
  • Stage 1 hypertension: 130–139 systolic or 80–89 diastolic

Urinary Tract Infection (UTI)

Overview

A urinary tract infection happens when bacteria enter the bladder or urinary tract, causing burning with urination, urgency, frequent urination, or pelvic discomfort. Most UTIs involve only the bladder and are uncomfortable but not dangerous when treated promptly.

Overview

Vaccines are one of the most effective tools in medicine for preventing serious childhood illness. The routine immunization schedule protects children against diseases that, within living memory, disabled or killed thousands of children every year in the United States — including measles, whooping cough, and bacterial meningitis.

Key point: On-time vaccination doesn't just protect your own child — it protects infants too young to be vaccinated, and people with weakened immune systems who can't be vaccinated at all.

Whose Guidance We Follow

Vaccine guidance has become more complicated to follow. In 2026, the federal government's official CDC vaccine schedule was revised to move several previously routine childhood vaccines into a "shared clinical decision-making" category, without the standard public review process the schedule has used for decades. The American Academy of Pediatrics and the American Academy of Family Physicians — the professional organizations representing the nation's pediatricians and family physicians — did not adopt these changes. Both organizations published their own 2026 schedules maintaining the prior, evidence-based recommendations, a position also endorsed by more than 200 other medical and public health organizations, including the American Medical Association.

Primary Care Curated follows the AAP and AAFP schedules, consistent with the rest of this site: we rely on primary care's own professional bodies rather than treating every source as equally authoritative. If your child's vaccine record differs from what your school or state requires, talk to your child's doctor — school and state requirements are set separately from these medical recommendations.

The Immunization Schedule

The AAP and AAFP schedules cover vaccines from birth through age 18, given across a series of visits: several vaccines in the first two years of life, boosters in early childhood, and additional vaccines and boosters around ages 11–12. Because exact ages, doses, and catch-up timing vary by vaccine and by each child's health history, we don't reproduce the full schedule here — your child's doctor follows it during well-child visits. Vaccine Safety

Vaccines used in the U.S. go through years of testing before approval, and their safety continues to be monitored afterward through federal systems including the Vaccine Adverse Event Reporting System (VAERS) and the Vaccine Safety Datalink. Large, independent studies involving millions of children have found no link between vaccines and autism. Most side effects are mild — soreness at the injection site, a low fever, fussiness — and resolve within a day or two. Serious reactions are rare, and are exactly why safety monitoring continues for as long as a vaccine remains in use.

Why Vaccination Rates Matter

By the numbers:
  • Measles requires roughly 95% community vaccination to prevent outbreaks ("herd immunity").
  • National kindergarten MMR vaccination fell from 95.2% in the 2019–20 school year to 92.5% in 2024–25, while exemptions from vaccination requirements rose to an all-time high.
  • The U.S. recorded more measles cases in 2026 than any year since 1991 — surpassing 2025, which had itself been the worst year in more than three decades.
  • Most 2026 measles cases have been among children and teenagers, the large majority unvaccinated.
  • The U.S. is now at risk of losing the measles elimination status it earned in 2000.

These aren't abstract numbers — they reflect real outbreaks spreading in communities where vaccination rates have fallen below the threshold needed to stop them.

Seek prompt medical care if your child has a high fever, rash, cough, or red/watery eyes and has been exposed to someone with measles — especially if they're unvaccinated or you're unsure of their vaccination status. Call ahead before arriving so the office can take precautions to avoid exposing other patients.

Common Questions

Do vaccines cause autism? No. This has been studied extensively across large populations in many countries, including studies involving more than a million children, and no link has been found. It remains one of the most thoroughly researched questions in modern medicine.

My child is behind on their vaccines — can we still catch up? Yes. Catch-up schedules exist for nearly every vaccine; talk to your child's doctor about a plan.

High Cholesterol

Overview

Cholesterol is a fat-like substance carried through the blood by particles called lipoproteins. LDL cholesterol (often called "bad" cholesterol) contributes to plaque buildup inside artery walls; HDL cholesterol ("good" cholesterol) helps clear it. High triglycerides, another type of blood fat, also raise cardiovascular risk. High cholesterol itself causes no symptoms — it's discovered through blood testing, not how you feel.

Diagnosis & Screening

A blood test called a lipid panel measures total cholesterol, LDL, HDL, and triglycerides.
